The Bridge to Health Webinar Series is a four-part educational program designed to help healthcare professionals better understand and care for vulnerable, high-utilizing patients who frequently cycle through healthcare systems.

Building on the Bridge to Health Symposium, the series goes beyond traditional trauma-informed care to examine patient experiences, provider biases, behaviors, and system-level barriers that influence care. Through real-world case studies, national experts, and practical tools, participants will explore the complex factors underlying conditions such as trauma, severe mental illness, substance use, autism, homelessness, and poverty.

The series equips clinicians, leaders, students, and care teams with strategies to build trust, improve engagement, reduce barriers, and strengthen outcomes. Ultimately, it aims to establish a shared, relationship-centered framework for managing complex patients with greater empathy, effectiveness, and collaboration while supporting population health and value-based care goals.
