TQC PSO Members Only: Safe Table on Handoffs and Transitions of Care

Handoffs occur frequently between care providers and care departments as a patient transitions through the care continuum, for example, from EMS to hospital ED to ICU to Med/Surg floor to skilled nursing facility. While on the surface, hand-off appears to be a simple process, but the successful transfer of information is complex and critical to patient safety. Recently, The Joint Commission acknowledged the importance of high quality hand-offs in its Sentinel Event Alert #58. Despite structured tools and frameworks, tackling the handoff process can be overwhelming. During this Safe Table we will discuss how you can use your event reporting system to identify contributing factors and help target your solution.What is a Safe Table? A Safe Table provides a forum for NCQC PSO members to network and exchange patient safety experiences and best practices in an open, uninhibited and legally protected environment. Safe Tables are legally protected under the Patient Safety and Quality Improvement Act of 2005 that allow health care providers to convene and have open dialogue about patient safety and quality of care issues. All attendees are required to sign confidentiality agreements.
